No matter where you are from no matter which chat room you enter at some stage while on Infertility chat or boards you will hear the word Clomid.Why does everyone talk about it and what is it ?
What is CLOMID??
 
Clomiphene citrate or commonly known as Clomid,Is one of the most popular fertility medications used by drs to bring on Ovulation in a woman.It is a synthetic drug that stimulates the hypothalamus,Producing more Gnrh, this then responds to the Pituitary to release more LH and FSH this is when the Ovarys stimulates and producing a mature egg.
Who benefits ??
 
Couples undergoing Fertility treatment such as IUI or IVF.
Women who have irregular cycles or short Luteal phases. LPD
Women with hormonal imbalances who do not produce enough lh or fsh.
Used for couples with Unexplained or secondary Infertility after lengthy testing.
Women who ovulate every second cycle is also a good candidate for clomid.
When do you take it ??
 
The regular course of dosage is usually between cycledays 3---7  or 5---9.
50 mg once aday. 5 days
If you ovulate on this dose there is no need to increase the dosage,Increasing un necessarily can infact cause problems such as cysts and multiple Ovulation additionally it can interfere with cervical mucas making it hostile or less amount, It can also bring on hot flushes.
 
If a women responds well to clomid and produced a mature follicle,but hasnt Ovulated by cycleday 15 a injection of  HCG can be given to bring on Ovulation this acts like LH to produce the release of the mature egg.Once lh surge or injection given you will usually Ovulate within the next 36 hours.
 
If however you do not Ovulate on clomid which can be determined by bloodtest and ultrasound,OPK test. Your dr will normally double the dose in the next cycle.
 
If after 6 cycles on clomid you do not achieve pregnancy this is a good time to talk with your dr and re think about other possibilitys of treatment. Such as IUI.
Monitoring
 
Women should be closely monitored while on clomid for Cysts and a Pct test should also be performed, Postcoital test this is when the cervical mucas is looked at after having intercourse, This is very simple and painfree.
 
Advantages for women with Luteal Phase defect LPD
Clomid used for women with LPD has been very effective, Although a woman may begin what appears to be a good cycle and even Ovulate when she enters the luteal phase (days between O and period) the lining of the uterus becomes disruptive due to small amount of progesterone being supplied, This interferes immensly on Implantation for the egg to attach itself to the wall cavity, Even if the embryo has implanted mentrastion is brought on early and ending the pregnancy and cycle.
How can this be helped ??
 
Using online charting which is free from Lifecyclesoft.com and taking your bbt everyday will help determine how long your luteal phase is. You count the days between Ovulation and the start of your next cycle, If this number is less then 13 then you should mention this to your dr on your next visit. This is commonly referred to as your LP.
Clomid can help the luteal phase alot as it stimulates the hypothalamus and the pituitary so they keep producing the hormones the ovarys need to supply progesterone for the luteal phase.Thus increasing the LP.and giving a greater chance for more time for the fertilised embryo to attach itself during implantation.
 Statistics of success
 
About 80% of women using clomid for poor ovulation will Ovulate and a staggering 50% will become pregnant within 6 months.
There is a small risk of multiples while using Clomid.
 
General Overview use and abuse
 
Clomid is known for being a safe drug when used correctly.
Your ovaries can get over stimulated and thus the need for careful monitoring.
Also there are minor side effects which can happen such as hot flushes, Tender breasts,Migraine,
Blurred vision. If the side effects are very severe then you should contact your dr to make sure you are ok to continue the treatment.As with all medication always read the lable and go by your Drs directions.
